Main Duties:
- Scrutinizing patient treatment sheets and prescriptions for suitability of prescribed drugs in relation to dosing accuracy, drug interaction and route of administration.
- Liaises with prescribing doctors to ensure rational use of medicines
- Ensures timely and accurate posting of all pharmacy computer transactions for billing purposes.
- Ensures timely orders for stock replenishment are made to maintain optimal stock levels.
- Responsible for drug information to patients, doctors, nurses and other medical staff.
- Responsible for storage and dispensing of narcotics and psychotropic substances in line with legal requirements.
- Participating in clinical ward rounds.
- Counselling patients appropriately to ensure adherence to drug therapy.
- Participating in quality management system and patient safety activities, including documentation, analysis, and
- reporting for the continued improvement of services.
- Reporting and documenting adverse drug reactions and medication errors in the Hospital.
- Receiving, evaluating and making recommendations on applications for the inclusion/ deletion of drugs into the Hospital formulary on behalf of Medicines and Therapeutics Committee (MTC).
- Participate in monitoring, evaluation and reporting on the use of medicines in the Hospital by
- producing quarterly and annual drugs utilization reports.
- Interpreting clinical evidence to advise on and endorse drug protocols.
- Interpreting clinical evidence and individual patient information to advise on and endorse patient care.
- Using critical appraisal skills in the review of clinical and cost efficiency data to promote clinical
- governance in the use of medicines
- Managing and reconciling conflicting opinions of professionals to optimise the use of medicines
- Participating in working groups /committees as requested.
- Participating in quality improvement initiatives within the department and providing reports as required.
- Any other duty assigned.
Required Qualifications for the Position
- BSc. in Pharmacy.
- A Masters in a healthcare-related field will be an added advantage.
- Registered with the Pharmacy and Poisons Board with a valid current practising license.
- 3 years post-internship experience.
- Keen on detail with a high degree of accuracy.
- Good communication skills.
- Supervisory skills.
- Computer skills
