Job Objective/Purpose
The Program Lead for Chronic Care and Wellness is responsible for overseeing, designing, and managing programs that address chronic health conditions and promote wellness initiatives.
Key Responsibilities
Program Development and Management
- Design, implement, and oversee chronic care and wellness programs aimed at improving patient outcomes and overall health.
- Develop and refine program strategies to ensure alignment with organizational goals and patient needs.
- Collaborate with multidisciplinary teams to integrate wellness initiatives and chronic care services seamlessly.
- Establish, track, and report on program performance metrics, utilizing data to refine and enhance program offerings.
Patient and Community Engagement
- Serve as the primary point of contact for patient outreach and engagement in chronic care and wellness programs.
- Build relationships with community organizations and healthcare providers to increase program accessibility and impact.
- Conduct educational workshops, webinars, or health fairs to promote wellness and disease prevention.
- Gather feedback from patients, caregivers, and providers to identify areas for improvement in program services.
Healthcare Coordination
- Coordinate with healthcare providers and other team members to ensure a cohesive approach to chronic disease management and wellness.
- Support the creation of individualized care plans, promoting adherence to wellness goals and treatment regimens.
- Facilitate patient access to services such as nutrition counseling, physical activity resources, and mental health support.
- Oversee case management for high-risk patients, ensuring consistent monitoring and follow-up on their care plans.
Person Specification
- Bachelor’s degree in Nursing/Public Health/ Healthcare Nursing,
- A valid Nursing Council of Kenya license
- Minimum 3 years experience
- Attention to detail.
- Excellent interpersonal skills and a team player
